What are Wellness Services vs. Physical Therapy Services?
Physical Therapy Services are prescribed by your physician and typically covered by your health insurance. Wellness Services at Lafayette Physical Therapy are a group of services for healthy living. We offer:
- Wellness Physical Therapy: Private Pay Physical Therapy for individuals who do not wish to use their insurance, have an HMO or other insurance not accepted by Lafayette Physical Therapy, or individuals who do not have insurance. We also have Post-Prescription Physical Therapy for individuals with insurance that covers a limited number of visits or does not consider the treatment medically necessary but the individual still wants to continue.

What should I bring with me to my initial evaluation appointment?
- Prescription for physical therapy from your Doctor
- Insurance Card(s)
- Completed Paperwork (If able to print from website)
- Wear comfortable clothes that allow the therapist to work with the part of the body you will receive treatment for.

How long does each appointment take?
An evaluation normally takes one hour. Each subsequent visit takes between 45 minutes and an hour depending on your treatment and exercise plan.

What should I wear to my physical therapy visits?
Comfortable clothing that will allow your therapist to access the injured body part and practical shoes. You may bring clothes to change into before your appointment.

How often do I need to come to physical therapy?
Frequency and duration is dependent on your doctor’s recommendations. Most commonly it will be stated on your initial prescription from your physician. If the prescription states “Evaluate and Treat”, then your therapist, upon evaluation, will determine the frequency and duration necessary to address your issue(s). Depending on your condition your therapist may request additional visits and/or alter the frequency of the appointments.
The total number of therapy visits is also dependent on the maximum physical therapy visits allowable per year by your individual insurance plan.

What if I need to cancel an appointment?
We require 24 hour notice of any cancellations to avoid a $65 cancellation/no show fee. We also appreciate notice of cancellations of Monday appointments by 3pm the Friday before.
We understand that you may need to change an appointment every once in a while. Please give us adequate notice and have your schedule available to re-schedule your appointments so you will not miss any prescribed visits.
Workers Compensation patients cannot be charged for missed or late cancellations. However, the primary physician and the case manager will be notified.

What types of insurance do you accept?
We are preferred providers for most private insurances including: Blue Cross, Blue Shield, HealthNet, United Healthcare, Aetna, Great West, Cigna, and many other Private Insurances. While we accept Medicare, we do not accept MediCal or HMO insurances. Some workers compensation insurances are accepted. Please call to verify.

What is the difference between a deductible and a coinsurance?
A deductible is the share of cost you are expected to pay before your insurance begins to pay your claims.
Co-insurance is a percentage of the cost for each visit you are seen for. Your insurance pays a percentage and you pay the remaining percentage. Example: Insurance Pays 70% and you pay 30%.

How much of my treatment is covered by my insurance company?
Your patient responsibility is dependent on your individual plan and may vary greatly from one patient to another.
You may find this information by logging onto your insurance website, calling the Customer Service Line listed on the back of your insurance card, or by reading your insurance coverage manual issued by your insurance carrier.
LPT will verify your physical therapy benefits prior to your initial evaluation, and we will be able to discuss those benefits with you when you check in for your first visit.

How will I be charged or billed?
If you have a deductible, we will collect a minimum of $50 each visit until it is met. If you have a copay we will collect it before each visit. We send statements once a month for any balances that will include coinsurance payments and other balances uncollected.

Do you bill Secondary Insurances?
As a courtesy to our Medicare patients we will bill one secondary insurance. We strongly recommend that all Medicare patients register their secondary insurance with Medicare to ensure timely processing.

What if I cannot pay?
Our billing department can develop a payment plan with patients who are unable to pay their full balance upon receipt of a statement. This plan will allow patients to continue treatment while paying down their account without interest. If you expect to have a difficult time paying your balance you must enroll in a payment plan with our accounts manager prior to continuing treatment.
If you do not pay your account balances and do not enroll in a payment plan your account may be subject to interest and turned over to an external collection agency. Lafayette Physical Therapy is contracted with a reputable collection agency. Please stay in good communication with our office to ensure that your account is not turned over to collections.

How much does physical therapy cost?
We have contracted fee schedules with many insurances. Fee schedules depend on each individual's insurance. It is not always possible to determine exactly how much your insurance will be billed due to the nature of medical coding. Your insurance will be billed based on the different types of physical therapy treatments provided.

Payments
When do I need to pay you?
Payment Requirements depend on individual insurance plans. General guidelines include: - Copayments are due at the time of each visit.
- Patients who have a deductible will be required to pay a portion of the deductible for each visit. The patient will be billed for the difference once the insurance is billed and the insurance company assigns responsibility to the patient.
- Co-Insurance is due upon receipt of a statement
- Any balances will be billed out on a monthly basis and are due upon receipt.
- Private Pay patients are required pay for visits prior to receiving treatment."
